Medica Superspecialty Hospital, the leading private hospital chain in Eastern India, has achieved a groundbreaking achievement by successfully conducting the first-ever 'Zero Contrast Rotablation Angioplasty' in the region. 

The procedure was performed on a 63-year-old male, Prashanta Bhattacharya, a retired executive, resident of Behala, by Dr. Arindam Pande, Senior Consultant in the Department of Cardiology, along with the collaborative efforts of Dr. Nandini Biswas, Senior Consultant Pulmonologist, Dr. Rohit Rungta, Senior Consultant Nephrologist, and the Cardiology Cathlab Team at Medica Superspecialty Hospital on 9th December 2023.

Prashanta Bhattacharya had been experiencing worsening shortness of breath and chest discomfort, eventually leading to orthopnea, severely impacting his quality of life. He had a history of long-term diabetes and compromised kidney function.After admission, evaluation revealed he had an Inferior Wall Myocardial Infarction (heart attack) alongside reduced heart pumping. 

His angiography disclosed critical narrowing of the right coronary artery compounded by severe calcifications, necessitating urgent intervention. Conventional Contrast Angioplasty posed a risk of further deteriorating kidney function, prompting the decision to proceed with the innovative Zero Contrast Angioplasty technique guided by Intravascular Ultrasound (IVUS) and incorporating Rotablation technology to manage coronary calcium. Post-procedure, Prashanta Bhattacharya experienced improved symptoms and a partial recovery of kidney function.

Dr. Arindam Pande, commented, "Managing this case posed a unique challenge due to the patient's diabetic condition and impaired kidney function. While Coronary Angioplasty with stents is a well-established intervention, the use of contrast dye in conventional procedures can aggravate kidney issues. 

The Zero-contrast Rotablation Angioplasty, a specialized technique utilizing Intravascular Ultrasound and Rotablation, is designed to treat severely calcified coronary artery lesions without the use of contrast dye. 

This approach eliminates the risk of kidney damage or allergic reactions, making it particularly beneficial for patients with kidney problems or contrast dye allergies. We are proud to announce the success of this procedure for the first time in Eastern India, with the patient's swift recovery and resumption of daily activities."

Sharing his experience, Prashanta Bhattacharya said, "My admission at Medica was timely with my deteriorating health. It may worsen urea and creatinine levels and consequently the patients with kidney problems may get complicated. When heart disease and kidney problems coexist, doctors perform angioplasty with minimum contrast or often no contrast which is known as Zero-Contrast Angioplasty. 

But Prashanta Bhattacharya's case was remarkable in the sense, that Dr. Arindam Pande and his team performed Intravascular Ultrasound (IVUS) guided Rotablation with angioplasty, which is a very complex coronary intervention. 

In this process, the calcium of the coronary artery is removed as well as the blockage of the artery is being cleared, without using any contrast. Report of such Zero-Contrast angioplasty with Rotablation is seldom accomplished in India. 

It requires a highly skilled cardiologist with Intravascular Ultrasound (IVUS) experience and Rotablation. My heartfelt congratulations to Dr. Arindam Pande and his team for such an outstanding achievement in patient care."
